• Dental BeneFits Plan - PPO coverage from Anthem Blue Cross Life and Health Insurance Company - free cleanings, exams, and X-rays and other significant savings when you visit one of Anthem Blue Cross' 12,500 contracted dentists...this plan is inlcuded in the Hospital BeneFits Preferred Plan.



• Dental Net Plan - HMO coverage from Anthem Blue Cross - coverage for a wide range of Dental services, including orthodontics, from Anthem Blue Cross network providers.

Employers must contribute 50% or more (in 5% increments) to each employee's monthly premium. Contributions to employees' dependents' premiums are optional.h dental plan options your employees may choose from.
